Resident Evil Requiem has no shortage of puzzles to work through, and while some of them hand you the solution fairly quickly, others require you to hunt down clues scattered across the Care Center before you can even attempt them. Between the puzzle boxes, analyzer specimens or the Organ Transport Box, it is easy to get stuck if you miss a key item or wander into a room without the right context.

In this guide. I will provide you with a solution to every puzzle you run into in the game all in one place. So you can get through each one without backtracking or wasting time trying to decode clues you may have already walked past.

Solutions of All The Puzzles

There are a total of seven main puzzles (eight on Insanity difficulty) that you will need to solve in order to progress further in the game. The solution to each puzzle is given below.

Chairman’s Office Puzzle Box

The first puzzle that you will run into is the puzzle box located inside the Chairman’s Office. To solve it, simply interact with the puzzle box and input the following pattern:

  • Moon —> Sun —> Star —> Moon

This will open the box and will give you Moon Quartz.

Lead Researcher’s Office Puzzle Box

The next puzzle is inside the Lead Researcher’s Office. This is similar to the first puzzle. Interact with the puzzle box and input the following pattern:

  • Star —> Sun —> Moon —> Sun

Doing so will open up the puzzle box and reveal a Leatherbound Notebook and the Sun Quartz.

Organ Transport Box

You will eventually run into an Organ Transport Box. Inspect it and you will find a hatch at the back of the box which you can open. Opening it will reveal a set of buttons, knob and a slider. Follow the steps I have given below in the exact order to open up this box:

  1. Turn the knob from “AUTO” to “HAND“.
  2. Push the slider on the right to the middle point.
  3. Press the button labeled “5”.
  4. Press the button labeled “2”.
  5. Turn the knob back from “HAND” to “AUTO”.

If you followed the steps exactly as I mentioned them, the box will open and reveal a sealed pair of Artificial Lungs.

Analyzer Blood Specimen Puzzles

You will run into a couple of different types of Blood Specimen which you need to analyze in the Blood Lab. The analysis requires solving a puzzle on the computer screen inside the game.

Blood Specimen (Denatured)

The first Blood Specimen called Denatured is located right inside the Blood Lab. Pick it up and then interact with the analyzer. Select the Blood Specimen (Denatured) to start the puzzle. Simply click on the second node from the left to solve the puzzle.

Blood Specimen (Converged)

The second Blood Specimen labeled Converged is found inside the Ward Office. To get inside, you will need ID Wristband (Level 1) which you can pick up from a dead scientist in the hallway near the Lead Researcher’s Office.

When playing on the Insanity difficulty, the Blood Specimen (Reversible) is found in the Records Room instead of the Ward Office.

Once you have it, put it in the analyzer of the Blood Lab to start the puzzle. To solve this puzzle, click on the top middle node and then the second node from the bottom.

Blood Specimen (Reversible)

The third Blood Specimen labeled Reversible is found inside the locked room of the Blood Lab. You can open this room with ID Wristband (Level 1). Put the Blood Specimen (Reversible) in the analyzer to start the puzzle. To solve this puzzle, follow these steps in the given order:

  1. Click on the leftmost node.
  2. Click on the Middle node.
  3. Click on the rightmost node.
  4. Finally, click on the node between the middle and the rightmost node.

Blood Specimen (Polymerized) – Insanity Difficulty

The Blood Specimen (Polymerized) is available when playing on the Insanity difficulty. This is found in the Closet near the Waiting Room. You will need ID Waistband (Level 1) to unlock the Closet room. To solve this puzzle, follow the steps I have given below:

  1. Click on the left middle node.
  2. Click on the right middle node.
  3. Lastly, click on the middle node.

VIP Suite Double Helix Puzzle

The final puzzle that you will have to solve is located in the VIP Suite. Grab the wine bottle present on top of the table to your left when you enter the room. Rotate the top of the wine bottle to match the label design. Once it matches, it will reveal the Double Helix Model.

In the middle of the room, you will find a standing wooden music box. Interact with it to reveal the puzzle. Put the Double Helix Model in the middle and the puzzle is complete.

That covers every puzzle solution you will run into across Resident Evil Requiem’s main story. Most of them are satisfying to work through once you have the right context, but a few, particularly the Blood Specimen analysis and the Organ Transport Box are genuinely easy to fumble if you do not know the exact steps ahead of time.

With all the main puzzles out of the way, you can focus on everything else the game throws at you. If you are still working through the Care Center or pushing further into the story, the solutions above should keep you from losing progress on anything puzzle related.
