Resident Evil Requiem – How to Open Organ Transplant Box, Puzzle Guide

Finding a cold, locked metal box in a freezer van is peak Resident Evil logic, and in Resident Evil Requiem, the Organ Transport Box is your ticket to the next level of security clearance.

You’ll find this “medical souvenir” in the Garage of the Rhodes Hill Critical Care Center, but getting it out—and actually getting it open—requires a bit of steady aim and a trip to the Director’s Office. Here is the breakdown on how to crack it.

Step 1: The Garage “Welcome” Party

The box is sitting in the back of an open freezer van in the Garage.

• The Trap: The moment you pick it up, an infected zombie is going to crash a construction vehicle through the opposite wall. Don’t panic and try to run; you won’t make it.
• The Fix: Line up a single shot to the driver’s head. One bullet is all it takes to stop the vehicle in its tracks. Once the wreckage settles, you can climb over it and head back inside.

Step 2: Grabbing the Instructions

Here is the catch: even if you know the code, the game usually won’t let you interact with the box’s panel until you’ve “found” the instructions.

1. Head to the Director’s Office.
2. You’ll need Corrosive Acid (found in the room directly opposite) to gain access to the safe/binder area.
3. Look for a Red Binder (or Leatherbound Notebook). Inside, you’ll find the file “Unlocking the Organ Transport Box.”

Step 3: Solving the Box Puzzle

Take the box into your inventory, select it, and choose Examine. Rotate it to the back side to find the control panel. If you’ve already messed with the buttons, hit Reset first to make sure you’re starting fresh.

The 5-Step Sequence:

1. Switch to Manual: Rotate the circular dial from AUTO to HAND.
2. The Slider: Move the slider on the far right from the bottom up to the middle line.
3. Button 5: Press/Activate the Number 5 switch.
4. Button 2: Press/Activate the Number 2 switch.
5. Back to Auto: Rotate the circular dial from HAND back to AUTO.

Final Steps

If done correctly, the box will hiss open, revealing a pair of Artificial Lungs. Now that you have the lungs, head to the Medical Examination Room. There is a body there that is… well, let’s just say it’s missing a few parts.

Insert the lungs (along with the other required organ) into the body and as for the rest… we’ll avoid the spoilers!

Opening the Organ Transport Box is a classic “stop and think” moment in the middle of a high-stress garage escape. It’s a bit of a process—acid, notebooks, and specific dial turns—but those Artificial Lungs are the only thing standing between you and the Level 2 areas. Just remember: headshot the driver first, ask questions about medical ethics later.
