The Delirium content is one of the endgame content that got a major rework in patch 0.5 of Path of Exile 2. It now has a questline that you can follow to easily progress through the content itself.
Not to mention, there are some changes added to the mechanics as well. In this guide, we’ll take a look at the changes in Delirium in patch 0.5 and how to do the content comfortably. Let’s get started.
How Delirium Works in Patch 0.5 of Path of Exile 2
It’s worth noting that Delirium is one of the hardest endgame content in the game. After all, the mechanics themselves make map monsters and bosses even tougher and stronger than they already are.
Of course, the harder the content, the more rewarding it can be. Delirium drops Liquid Emotions, which can fetch high prices on the market, with some selling for a few Divines each.
Hence, if you plan on doing the content and farming it, make sure you have a strong build in the first place. With that in mind, how exactly does Delirium work?
Delirium Content Mechanic

While mapping, you’ll be able to come across this structure that looks like shards of a broken mirror at the start of the map. This will mark the start of a Delirium encounter, and all you have to do is approach the mirror and pass through it.
Doing so will then spawn a fog that encompasses the entire map, as well as the Delirium gauge at the bottom of the screen.
When it’s your first time encountering Delirium in maps, you will come across an NPC named Elder Madox. He’ll be the one to introduce you to the newly added Delirium questline. He’ll then ask you to travel to The Withered Willow on the map.

As you get close to the area, each nearby surrounding map will have a Delirium encounter. Completing each encounter successfully will then grant you an Atlas skill point, which you can then use to fully unlock your Atlas tree.

The bar will indicate how deep into the Delirium you are, and the deeper you get, the harder the encounter becomes.
Simply follow the direction of the fog to have an idea of where you should go. The indicator at the bottom of the bar corresponds to whether you are heading in the right direction or not.
However, you have to keep in mind that the fog won’t last indefinitely. It will eventually dissipate, and once it catches up to you, the Delirium encounter ends.
Your final destination is the boss of the map, which you have to defeat while the Delirium fog is still there. Only then will you be able to complete the Delirium encounter.
What to Do During a Delirium Fog
There are some unique encounters that you can only come across while inside a Delirium fog. As you progress, you’ll notice these shards of glass lying on the ground. Walking on them will spawn a pack of Delirium monsters.
The most common are the green shards, which will spawn a pack of stronger Delirium-exclusive monsters.

Then there’s the pink shard that spawns a pack of powerful monsters, which in turn will drop more rewards when slain.
Occasionally, you’ll also stumble upon a red shard. Approaching it will send you to a different dimension, and your goal is to escape that area while dealing with Delirium monster packs.
Successfully doing so will grant you a powerful amulet that comes with two instilled Notables.
Progressing the Delirium Questline
Once you make it to The Withered Willow, Elder Madox will then provide you with instructions on what to do next.

A nearby area will then open on the Atlas, revealing a bunch of maps containing a Delirium encounter. Once you have completed one of the maps, a Grand Mirror will then appear.

This will cause the map boss to have a duplicate, hence forcing you to fight two bosses at the same time. If you manage to complete the map, a bunch of nearby maps will have permanent Delirium in them.
This means that the fog won’t disappear, and the encounters will be a lot harder. Unlike in a normal Delirium map, a map with a permanent Delirium fog will have slightly different mechanics.

For example, instead of the typical Delirium bar, you will now have a progress bar that fills up whenever you run a Delirium map with permanent fog. However, you won’t be able to fill it up in a single map.
You’ll have to run multiple maps to fill it up all the way to 100%. Once filled up, you will then unlock the Simulacrum. You can still push it all the way to 200% for even more rewards but also even harder encounters.
Running the Simulacrum

With the Simulacrum unlocked, a new map called the Simulacrum of Delusion will then appear on the Atlas. This will be different than the standard Delirium map.
This time, you will enter an area filled with waves upon waves of Delirium monsters. There are a total of 7 waves you must complete, with each wave providing different rewards but also increasing the difficulty until the final wave.
To start, simply click on the highlighted object on the ground. All you have to do is survive and kill all of the monsters that spawn.

Once you beat the first wave, you’ll then move to another room nearby which will contain the next wave. Rewards will also drop as soon as you complete a wave.
You’ll have to keep doing this 7 times.

If you manage to clear all 7 waves, you will then get the Raven’s Reflection quest item. This allows you to challenge the Pinnacle boss of Delirium, Tangmazu the Raven Trickster.
As a Pinnacle boss, you can expect this fight to be a really hard one. So, unless you have a build that can survive Tier 15 maps with no problem, I suggest setting this fight aside until you’re ready.
Simply take the Raven’s Reflection to The Withered Willow. Interact with the withered tree and place the item to open the arena leading to the fight.
After completing the Delirium questline, Grand Mirrors will then appear across the Atlas once you complete Delirium encounters.
You then have the option to choose where you want to place the Delirium fog on the Atlas. At the same time, you will get to fight an even stronger version of Tangmazu for even more exciting rewards.
How to Farm Delirium Content after the Questline
The main thing you want to farm from Delirium is the Liquid Emotions. Not only are they easier to get compared to the other stuff, most of which are from Tangmazu, but some of them also sell at a high price.
Moreover, you’ll be using some of them to anoint your amulet.
One of the most effective strategies I’ve seen involved running Delirium maps and boss rushing through them. For this, you’ll need 1x Delirium tablet with stack size of splinters and effectiveness, 1x Delirium tablet with chance for more unique bosses, and 1 Overseer tablet.
For this, you want to be running maps in the City biomes only.
Final Thoughts
Personally, I found the Delirium content in Path of Exile 2 to be one of the worst. Fortunately, the rework has made it one of the most fun and rewarding pieces of content in the game right now.
Of course, you have to make sure you can survive the maps with 100% deliriousness or even more. And for that, you need a powerful build that can handle all the content in the game with ease.
