You can find the entrance to The Grim Tangle from The Grelwood. As we’ve mentioned before, though, its exact location is completely random. It changes every time you go into a new Instance, too.
The easiest way to find it is to simply explore the area while looking out for fungal enemies, primarily the Fungal Zombies. Once you find them, you want to try paying attention to which direction they came from and follow it.
You’ll also want to look for yellow glowing pustules on the ground. You should start seeing them more frequently after you first encounter Fungal Zombies, and they become increasingly common as you get closer to The Grim Tangle.
Keep following the trail of Fungal Zombies and pustules until you eventually find a hole covered in mushrooms and pustules. That’s the entrance to The Grim Tangle itself, so you just need to enter it!
