To get to the secret cave, open your map and click on the Library of Zoltun Kulle as shown in the image above.
Then, teleport to the Grand Vestibule Waypoint where the secret cave is located.
Finally, search your surroundings for Chests, and kill Minions at the same time. There is a chance that a Legendary Item will drop from this area.
Keep an eye for Lost Pages so you can access the secret cave. Once you collect 5 of them, you can create a portal and enter the hidden cave, where valuable loot and more Legendary Items await.
