Also, take note that there are other altars you can take advantage of in certain areas, such as the Weynon Priory that you go to in the very first main quest.
The main city chapels are just the most convenient for the most part as you can easily fast travel there and spot the chapels from afar.
In addition to the fame and infamy mechanics, you should also ensure that you have no active bounty. If you somehow manage to reach one of these altars as a wanted person without attracting the attention of guards, you will not be able to use it.
That is pretty much everything you need to know in order to cure your diseases for free, with a slight catch. You can even use this to prevent vampirism as it can clear the diseases that leads to it, though you still will not be able to cure yourself if you fully turn.
