In order to get the Ballistic Vest cosmetic item, you want to head over to Wearhard Industrial Clothing in Smelterville East. As soon as you get there, look for the merchant selling stuff outside of the building. Approach the merchant and browse his collection of items.

There, you can find the Ballistic vest which you can buy for only $1,800. All you have to do is purchase it and wear it on your character.
